I.D. is required at time of pick-up. In the event that someone other than yourself will be picking up your medical records a signed release is required. Please have this signed form present at time of pick-up.
RAI is now offering a FILM PICK UP SERVICE to our patients. We will arrange to have your outside mammography films picked up locally, including Princeton, and delivered to our imaging center in time for your appointment. Please print and sign our Record Release Form and fax it to 585-1825. This form must be returned before pick up can be arranged.
